Post-Operative Instructions Following Periodontal Surgery

The first days are about protecting the surgical site.

Minor bleeding or oozing, swelling and bruising can occur after periodontal surgery. Avoid disturbing the area and follow the personalised instructions given by your treating clinician.

First 24-48 Hours: What to Avoid

Do not drink through a straw, spit forcefully or rinse vigorously. Avoid hot, spicy, crunchy or citrus foods; strenuous exercise; smoking and alcohol. Do not touch the surgical area with your fingers or tongue, and avoid repeatedly pulling the lip or cheek to inspect the site.

Do not brush or floss directly over the operated area during this initial period unless your clinician has instructed otherwise. Avoid an electric toothbrush at the surgical site until you are advised that normal cleaning can resume.

Bleeding

A small amount of bleeding or oozing can be expected. If bleeding occurs, place damp gauze over the area and maintain gentle, steady pressure for approximately 15-20 minutes while resting quietly with your head elevated. If significant bleeding persists despite pressure, contact SR Dental Zone.

Swelling & Bruising

Apply an ice pack externally during the early post-operative period as instructed. The source guidance recommends approximately 20 minutes on and 10 minutes off during the first four hours. After the first 24 hours, warm moist heat may be used if advised and if swelling or bruising remains uncomfortable.

Rinsing From the Following Day

Beginning approximately 24 hours after surgery, gently rinse with lukewarm salt water as directed. Do not rinse forcefully. If a medicated mouth rinse has been prescribed, use it according to your clinician's instructions rather than adding or substituting other products on your own.

Pain & Medication

Some discomfort is expected after periodontal surgery and may fluctuate during the first few days. Take prescribed medication exactly as directed. If antibiotics have been prescribed, follow the prescribed course unless your treating doctor advises otherwise. Contact the clinic if pain is severe, progressively worsening or not controlled by therecommended medication.

Food & Hydration

While numb, avoid chewing and choose room-temperature or cool, soft foods. Suitable options include yogurt, soft rice or khichdi, soft-cooked vegetables, creamed cereals, eggs, soft fish, stewed chicken and soups that are not hot. Do not use a straw. Once numbness has completely resolved, increase food texture gradually while avoiding the surgical area.

Protect the surgical site when eating.

Choose foods that require minimal chewing, chew away from the treated area where possible, and avoid hard, sharp, sticky or very hot foods until your clinician advises that normal eating can resume.

Smoking, Tobacco & Alcohol

The original document advises avoiding smoking and alcohol during the early healing period. Because tobacco and nicotine can adversely affect periodontal wound healing, longer abstinence is preferable. Avoid smoking, vaping, chewing tobacco, gutka, pan masala and other nicotine-containing products during healing, and discuss cessation support with your clinician if needed.

What Healing May Look Like

Tenderness, swelling, mild bruising and small amounts of blood-stained saliva can occur early after surgery. The appearance of the gum tissues may change as swelling settles and healing progresses. Do not judge the final gum contour or esthetic result during the early healing phase.

  • Bleeding that remains significant despite sustained pressure.
  • Rapidly increasing or concerning swelling.
  • Fever, persistent foul taste or discharge.
  • Severe or increasing pain not controlled by prescribed medication.
  • Loosening or displacement of a dressing, membrane, graft or sutures when these were placed.
  • Persistent numbness or any other unexpected change that concerns you.

If Sutures, Grafts or Dressings Were Used

Periodontal procedures vary. If sutures, a periodontal dressing, soft-tissue graft, regenerative material or membrane was used, do not pull, lift or repeatedly inspect the area. Follow the procedure-specific instructions provided by your treating clinician, as cleaning, diet and review timing may differ.

Your Follow-Up Journey

Within 72 hours: Schedule your first post-operative review within 72 hours of treatment completion.

During healing: Attend all additional reviews recommended by your treating clinician. These visits allow the surgical site, soft-tissue response and healing progress to be assessed and cleaning instructions to be advanced safely.

Long-term: Your periodontal maintenance interval should be based on your gum health and risk profile rather than assumed to be the same for every patient.
